FollowMyHealth Portal Instructions

If you are completely new to FollowMyHealth:

  1. Click this link: Click Here
  2. Click "I Need to Sign Up"
  3. Follow next steps in Sign Up and Connect to create your account.
  4. Log into your account.
  5. Add DDA to existing account:
    • Tap the Menu icon (usually three lines or "More") and select "My Account."
    • Choose "Connections."
    • Tap "Add Connection" (or the + icon).
    • Search for "Digestive Disease Associates" or use our zip code (32605) to find us.
    • Select our practice and follow the prompts to request a connection.

If you don’t remember your username/password:

  1. Click Sign In
    • Click “Forgot your password?” to reset your password.
    • Click “Forgot your username?” to reset your username.

FollowMyHealth Portal FAQ

I already have a FollowMyHealth account with another doctor. Do I need a new one for DDA?

No. You only need one FollowMyHealth account. You can easily connect your existing account to Digestive Disease Associates (DDA) to see all your health information in one place.

How do I add DDA to my existing FollowMyHealth app?

If you already have the app on your phone for a different provider, follow these steps to add us:

  1. Log in to your FollowMyHealth app.
  2. Tap the Menu icon (usually three lines or "More") and select "My Account."
  3. Choose "Connections."
  4. Tap "Add Connection" (or the + icon).
  5. Search for "Digestive Disease Associates" or use our zip code (32605) to find us.
  6. Select our practice and follow the prompts to request a connection.

I received an email invitation from DDA. What should I do?

If you received an invite, the process is even faster:

  • Click the registration link in your email.
  • When prompted, choose "Add this Connection" instead of "Create an Account".
  • Log in with your existing FMH credentials.
  • Enter your invitation code (typically your 4-digit birth year).

Why should I connect my DDA records to the portal?

Connecting to the portal allows you to:

  • View results: see your lab and pathology reports as soon as they are released.
  • Message the team: send secure questions directly to our physician’s Team Leaders reducing time spent on hold.
  • Manage appointments: view upcoming visits and request new appointments.
  • Access records: download your clinical summaries and medication lists whenever you need them.

What if I don't see my DDA information after connecting?

It can take up to 24–48 hours for your clinical records to sync once the connection is established. If you still don't see your DDA records after two days, please ensure that the name and date of birth on your portal account exactly match the information we have on file at the clinic.
