Appointment Reminders

As a courtesy, Digestive Disease Associates uses an automated service to remind you of your upcoming appointment. You will receive an email and/or text 7 days before your appointment with a link to complete your demographic and medical history.

It is important to complete these forms before your appointment to avoid a delay or reschedule. Remember to bring a copy of your driver’s license or picture identification and your insurance card. If you need to cancel or reschedule an office visit or procedure appointment, please notify us at least 48 hours in advance.
